Community Health Fee Schedule 2025-2026

As a community health service, Hesse Rural Health is funded by the State and Commonwealth Governments to deliver a range of community services to meet the communities health needs. We operate from community health centres situated at Winchelsea, Beeac, Bannockburn and Rokewood. We offer in-home, in-centre and virtual care appointments.

Hesse receives funding for a number of subsidised programs, including:

Home and Community Care (HACC PYP)

• Services for younger people (people aged under 65 years and Aboriginal people under 50)
• For clients who either don’t have a NDIS plan or the service request isn’t part of their NDIS plan
• Prioritises clients who are economically or socially disadvantaged and have complex care needs

Community Health

• A large range of universal services
• Prioritises clients who are economically or socially disadvantaged and have complex care needs

Commonwealth Home Support Program (CHSP)

• Services for older people (people aged 65 years and over and Aboriginal people 50 years and over)
• Referral required from My Aged Care, you can arrange a referral by calling the My Aged Care Contact Centre on 1800 200 422

Support at Home Fees Schedule 2025-2026 from 1st November

This is a summary of Hesse’s new Support at Home (SaH) service fees, not the co-contribution to be made by the individual participant. The co-contribution that an individual pays will depend on the service category and an individual’s means assessment/status, as determined by Services Australia.

CO-CONTRIBUTION

Under Support at Home, no participant will pay any co-contributions for clinical care services such as Allied Health and Community Nursing. How much a participant contributes to their support services for independence and everyday living will be based on their income and assets. The percentage of contribution to the cost of services will be:

PRICES FOR SUPPORT AT HOME SERVICES PROVIDED BY HESSE

The following prices are effective from 1st July 2025, and are hourly rates unless stated otherwise:

Travel

There is no fee charged for staff to get to your home. However individual transport is available to support consumers to attend appointments, shopping and social support activities. Hesse uses fleet cars and buses that are serviced, maintained and insured by the organisation. You can be assured of safe, reliable transportation. These trip costs are inclusive of the Hesse’s Health Care Staff time and the distance travelled.

CARE MANAGEMENT 10% of Support at Home Category

Hesse will allocate a Care Partner who will assist in meeting your needs through Support at Home. The Care Partner will develop a plan with you to support you to meet your goals. This includes:

Support with the implementation of your individualised care plan
Coordination of updates of your care plan as your circumstances change
Management of your invoicing and statements, for your quarterly budget
Ensuring that the requirements of your Support at Home Category are fulfilled

SUBCONTRACTED SERVICES (Associate Providers)

Participants wishing to contract individual external services specific to their needs can do so by negotiation through their Hesse Care Partner. Prices will be determined with the participant at time of agreement.

CANCELLATION

A minimum of 48 hours’ notice of cancellation applies or the full service fee will be charged.

PUBLIC HOLIDAYS

Public holiday rates apply to all Victorian public holidays, including those that fall on weekends.
