Two positions available for Case Manager Community Care:
Position 1: Temporary Part Time – 48 hours per fortnight, (4th April to 26th June 2022)
We are seeking a caring and qualified person to join our Community Care Case Management team on a temporary part time basis, to support the needs of older people to remain living at home. This temporary role is needed for 48 hours per fortnight from 04/04/2022 to 26t/06/2022 and is based from our Winchelsea campus.
Position 2: Permanent Part Time – 16 hours per week
There is also a permanent part time position available for 16 hours per fortnight.
The Case Manager Community Care roles involves assessment, planning and service coordination supporting consumer centred care delivery across Commonwealth Home Support and Home Care Packages programs and will suit a motivated, flexible person with well-developed planning, problem solving and decision making skills, who is comfortable working in a rural community and who thrives in a supportive team culture. The successful applicant will have diploma level qualifications in nursing, aged care or community care. A driver’s licence is required for travel throughout our service delivery area.
Public Sector conditions apply, salary packaging is available and fleet vehicles are provided for program delivery.
