Home Care Packages

A Home Care Package provides Government funded services that can help you remain at home for longer. Hesse is a recognised leader of Aged Care services with a 13-year history of providing Home Care Packages in the Barwon, Grampian and Colac Otway regions.

Our team of experienced and qualified staff provide services to help you remain living at home with access to flexible services and care.

You choose the type and manner of service delivery.

The types of services you can access under a home care package include, but are not limited to:

  • Personal services
  • Continence management
  • Mobility and dexterity
  • Transport and personal assistance.
  • Management of skin integrity
  • Nutrition, hydration, meal preparation and diet
  • Nursing, allied health and other clinical services: speech therapy, podiatry, occupational or physiotherapy services, hearing and vision
  • Telehealth
  • Assistive technology
  • Aids and equipment

Eligibility:

You may be eligible for a home care package if you are:

An older person who needs coordinated services to help you to stay in your home.

A younger person with a disability, dementia or other special care needs that are not met through other specialist services. (There are no minimum age requirements).

How can you get a Home Care Package?

You need to be assessed and approved for a package through My Aged Care.

We can assist you to do this or you can contact My Aged Care directly to discuss your needs.

Once approved for a Home Care Package, you can receive our services by contacting us directly.

To find out if you are eligible for a home care package or any other help at home services.

  • Call My Aged Care on 1800 200 422.
    The My Aged Care contact centre will ask you a series of questions to determine if you need an assessment:
  • Aged Care Assessment Team (ACAT) for a Home Care Package
  • Regional Assessment Service (RAS) Assessment, if lower level of needs can be met by the Commonwealth Home Support Program
  • Provide you with information about services that may assist you.

Fees

A list of Care Service charges is displayed on the My Aged Care website   or see Hesse Health Services Fees here. Non-pensioners may be required to pay an Income Tested Fee as assessed by Services Australia.
