Diabetes Education

The Diabetes service at Hesse provides education and support for people with diabetes and their families.

Our team is able to provide you with an introduction to diabetes self-management, help and support to manage your diabetes including education, blood glucose checking, monitoring your diet and medications.

We offer group and individual sessions depending on your needs.

Diabetes educators teach people how to recognise and treat symptoms and incorporate diabetes management into their lifestyle. Diabetes education is available on an individual or group basis.

Eligibility

All ages are eligible with a type 2 diabetes diagnosis or those who are of risk of diabetes.

Hesse can arrange an appointment at one of our community health centres or at your home to provide personalised support. Consultations can occur from Winchelsea, Beeac, Rokewood or Bannockburn.
