Community Nursing

Our community nursing team works across a 4,000 square kilometre area providing a comprehensive range of nursing and wellness services in the person’s own environment. Operating from the Winchelsea, Beeac, Rokewood and Bannockburn centres these services provide local care for local people.

The nursing team provides:

District nursing
Hospital in the home
Post acute care
Ante natal/ GP shared care
Post natal care
Palliative care

For over 65’s (over 50 for Aboriginal and Torres Strait people) referrals may be made by the individual, their family or other health care providers via the My Aged Care portal at http://www.myagedcare.gov.au/ or by phoning 1800 200 422.

All other referrals can be made directly to the Community Services Coordinator on (03) 5267 1200.
